GLP-1 Prices Are Dropping in 2026 — Here's What You'll Actually Pay

By TrimSoMo Editorial · · 2 min read

The single biggest GLP-1 story of 2026 is pricing pressure. After years of $1,000+ list prices, several cash-pay and direct-to-consumer paths have opened up. Exact prices, eligibility, and availability still change by product and channel, so treat the numbers below as orientation and verify before you pay.

1. The $150 oral pill (cash)

Some manufacturer cash-pay programs for oral semaglutide have been discussed around $150/month for eligible cash-pay patients. Confirm the current program, label, and eligibility directly with the manufacturer or pharmacy. (Remember the empty-stomach rule — see our oral GLP-1 guide.)

2. Direct-to-consumer injections (~$350/month)

New direct-to-consumer purchasing channels may bring some brand-name injectable GLP-1 cash prices closer to the mid-hundreds for eligible patients — versus the historical $1,000–$1,400 sticker. Medicare and Medicaid pathways are also shifting.

3. Telehealth memberships (still competitive)

Online clinics still bundle visit + medication + check-ins into a monthly fee. Brand programs commonly land $250–$550/month; the cheapest historically were compounded programs — but that landscape changed (next section).

4. Compounded: cheaper, but now restricted

Compounded semaglutide used to be the budget option. After the FDA declared the semaglutide shortage resolved in early 2025, compounding is now generally only allowed when a prescriber documents a specific medical need the FDA-approved drug can't meet. So the ultra-cheap compounded route is narrower than it was. More on compounded legality →

So what's the cheapest path for you?

Your situation Likely cheapest
Fine with a daily pill Manufacturer cash-pay oral route, if eligible
Want an injection, paying cash Direct-to-consumer brand route, if eligible
Have GLP-1 insurance coverage Your copay (often beats all of the above)
Need hands-on support A telehealth membership

Don't just chase the lowest number

The cheapest sticker isn't always the best deal. Ask:

  • Is medication included, or billed separately?
  • Brand or compounded? Which pharmacy?
  • Any consult fee on top?
  • What happens when my dose increases?
